Methods of Hemostasis"},"content":{"rendered":"\n<h2>Methods of Hemostasis<\/h2>\n\n\n\n<p>The surgical team can stop bleeding and achieve hemostasis in three unique ways, click on the boxes to learn more:<\/p>\n\n\n\n<p><strong>Mechanical<\/strong><\/p>\n\n\n\n<ol><li>Direct pressure such as holding a sponge or towel on a bleeding site&nbsp;<\/li><li>Suture materials&nbsp;<\/li><li>Ligating clips&nbsp;<\/li><\/ol>\n\n\n\n<p><strong>Thermal-Based Energy<\/strong><\/p>\n\n\n\n<ol><li>Electrocautery&nbsp;<\/li><li>Lasers&nbsp;<\/li><\/ol>\n\n\n\n<p><strong>Chemical<\/strong><\/p>\n\n\n\n<ol><li>Medications<\/li><\/ol>\n\n\n\n<p>Let us look at each of these <a id=\"post-451-_Int_MZM4RHwu\"><\/a>methods more closely.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h5>Methods of Hemostasis \u2013 Mechanical<\/h5>\n\n\n\n<p>Different mechanical tools can be used during surgery to provide temporary or permanent hemostasis.<\/p>\n\n\n\n<div class=\"slider\">\n    <div class=\"slide\">\n        <div class=\"slide-content\">\n            <div class=\"slide-image\" style=\"background: url('https:\/\/perioperative1001.slc-courses.ca\/wp-content\/uploads\/2022\/12\/word-image-451-13.jpeg');\"><\/div>\n            <p class=\"slide-description\"><strong>Hemostatic clamps&nbsp;<\/strong><\/p>\n            <p class=\"slide-description\">There are many different configurations of hemostatic clamps that can be used to occlude vessels and stop bleeding temporarily during a surgical procedure without causing significant trauma to the vessel.&nbsp;<\/p>\n            <p class=\"slide-description\">Clamps are used temporarily until a vessel can be anastomosed or repaired.<\/p>\n        <\/div>\n    <\/div>\n    <div class=\"slide\">\n        <div class=\"slide-content\">\n            <div class=\"slide-image\" style=\"background: url('https:\/\/perioperative1001.slc-courses.ca\/wp-content\/uploads\/2022\/12\/word-image-451-14.png');\"><\/div>\n            <p class=\"slide-description\"><strong>Clips<\/strong><\/p>\n            <p class=\"slide-description\">Clips can be placed across a vessel and then clamped to occlude and stop blood flow.&nbsp;&nbsp;Clips are more often used on larger vessels and are frequently made of titanium. These clips will remain in the patient permanently. There are polymer clips, as well, which will eventually be reabsorbed.&nbsp;Clips are often placed in pairs in case one becomes dislodged.&nbsp;&nbsp;&nbsp;<\/p>\n\n            <p class=\"slide-description\">For more information on hemoclips and appliers watch this video <a href=\"http:\/\/www.anatomyguy.com\/surgical-instruments-hemoclip-appliers\/\" target=\"_blank\" rel=\"noopener\">Hemoclip appliers&nbsp;&nbsp;Surgical Instruments: Hemoclip Appliers &#8211; Anatomy Guy<\/a><\/p>\n        <\/div>\n    <\/div>\n    <div class=\"slide\">\n        <div class=\"slide-content\">\n            <div class=\"slide-image\" style=\"background: url('https:\/\/perioperative1001.slc-courses.ca\/wp-content\/uploads\/2022\/12\/word-image-451-15.png');\"><\/div>\n            <p class=\"slide-description\"><strong>Ligatures (ties)<\/strong><\/p>\n            <p class=\"slide-description\">They are often called ties.&nbsp;<\/p>\n            <p class=\"slide-description\">They are strands of suture material without a needle that can be tied around a vessel to manually occlude the blood vessel.&nbsp;&nbsp;<\/p>\n        <\/div>\n    <\/div>\n    <div class=\"slide\">\n        <div class=\"slide-content\">\n            <div class=\"slide-image\" style=\"background: url('https:\/\/perioperative1001.slc-courses.ca\/wp-content\/uploads\/2022\/12\/word-image-451-16.png');\"><\/div>\n            <p class=\"slide-description\"><strong>Pledgets<\/strong><\/p>\n            <p class=\"slide-description\">These are small pieces of Teflon felt that either come already on a suture or are placed there by the scrub nurse. They are used when the surgeon is suturing a vessel and the hole from the needle causes bleeding. They are most often used in cardiac surgery.&nbsp;<\/p>\n        <\/div>\n    <\/div>\n    <div class=\"slide\">\n        <div class=\"slide-content\">\n            <div class=\"slide-image\" style=\"background: url('https:\/\/perioperative1001.slc-courses.ca\/wp-content\/uploads\/2022\/12\/image19.jpg');\"><\/div>\n            <p class=\"slide-description\"><strong>Packing<\/strong><\/p>\n            <p class=\"slide-description\">A surgeon may use gauze packing to maintain pressure on a wound surface to help control oozing.&nbsp;<\/p>\n\n            <p class=\"slide-description\">Packing must often be pre-moistened with warm saline. &nbsp;<\/p>\n        <\/div>\n    <\/div>\n    <div class=\"slide\">\n        <div class=\"slide-content\">\n            <div class=\"slide-image\" style=\"background: url('https:\/\/perioperative1001.slc-courses.ca\/wp-content\/uploads\/2022\/12\/image1f.jpg');\"><\/div>\n            <p class=\"slide-description\"><strong>Patties<\/strong><\/p>\n            <p class=\"slide-description\">These are small, compressed cotton sponges that are radiopaque and can be used for hemostasis on the brain tissue surface.<\/p>\n            <p class=\"slide-description\">They should be soaked before use with normal saline or an antibiotic solution.&nbsp;<\/p>\n        <\/div>\n    <\/div>\n    <div class=\"slide\">\n        <div class=\"slide-content\">\n            <div class=\"slide-image\" style=\"background: url('https:\/\/perioperative1001.slc-courses.ca\/wp-content\/uploads\/2022\/12\/word-image-451-19.png');\"><\/div>\n            <p class=\"slide-description\"><strong>Bone Wax<\/strong><\/p>\n            <p class=\"slide-description\">This is made from a sterile non-absorbable mixture which contains beeswax. Bone wax is used to provide a tamponade barrier on bone surfaces to stop oozing.&nbsp;<\/p>\n\n            <p class=\"slide-description\">It should be used sparingly, with any excess removed.&nbsp;<\/p>\n\n            <p class=\"slide-description\"> It can be rolled into small balls and presented on an elevator so the surgeon can apply it to bleeding bone edges.&nbsp;<\/p>\n        <\/div>\n    <\/div>\n  \n<\/div>\n\n\n\n<p>(Phillips &amp; Hornacky, 2020)<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h5>\ud83d\udcc1 Documentation Highlight<\/h5>\n\n\n\n<p>Clips used for hemostasis must be meticulously documented in the patient record because it is vital to keep track of any implants being left in the patient<a id=\"post-451-_Int_YmbPFtV5\"><\/a>. Why is this important? If the patient requires any diagnostic imaging in the future, including an MRI, the technicians need to be able to identify if there are metal implants and if they are MRI-compatible. Follow your institution\u2019s charting policies as to what is required for documentation.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h5>Thermal-based Energy<\/h5>\n\n\n\n<p>Different thermal-based equipment can be used during surgery to provide hemostasis.<\/p>\n\n\n<div class=\"h5p-iframe-wrapper\"><iframe id=\"h5p-iframe-18\" class=\"h5p-iframe\" data-content-id=\"18\" style=\"height:1px\" src=\"about:blank\" frameBorder=\"0\" scrolling=\"no\" title=\"Module 6: Thermal-based Energy\"><\/iframe><\/div>\n\n\n\n<p>(Phillips &amp; Hornacky, 2020)<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h5>Chemical Methods<\/h5>\n\n\n\n<p>Chemical methods of hemostasis work to react with blood to form a clot. <a id=\"post-451-_Int_cGwMSf09\"><\/a>Many of these chemical materials will re-absorb over time as the body heals. They do not have to be removed. There is one exception. Hemostatic chemical materials cannot be packed into a closed space where they could swell and cause pressure on surrounding tissues such as the spinal canal, or in <a id=\"post-451-_Int_uSGa526d\"><\/a>neurosurgery. In this instance, surgeons will still use a chemical agent <a id=\"post-451-_Int_siAu1yt6\"><\/a>but will carefully ensure it is removed before closing.<\/p>\n\n\n\n<p>Click on the various chemical hemostatic agents below to learn more about them. There are many quick videos to provide you with a visual and overview of the agent from their supplier. It is vital that perioperative nurses are knowledgeable about and follow the <a id=\"post-451-_Int_R6BHPkWX\"><\/a>manufacturer&#8217;s directions for hemostatic agents, <a id=\"post-451-_Int_OW8h8zft\"><\/a>sealants, and adhesives.<\/p>\n\n\n\n<p>(ORNAC, 2021)<\/p>\n\n\n\n<p><strong>Absorbable Gelatin&nbsp;(Gelfoam)&nbsp;<\/strong><\/p>\n\n\n\n<ol><li>It can come in a powder or foam pad.&nbsp;&nbsp;<\/li><li>It is an absorbable hemostatic agent.&nbsp;&nbsp;<\/li><li>It can be cut without crumbling.<\/li><li>When placed on a bleeding capillary, it swells and forms a large clot.&nbsp;<\/li><li>It may be soaked in a liquid such as saline, thrombin, or epinephrine, depending on the surgeon\u2019s preference.&nbsp;<\/li><\/ol>\n\n\n\n<p><strong>Oxidized Cellulose&nbsp;(Surgicel)<\/strong><\/p>\n\n\n\n<ol><li>It can be cut into strips and laid directly on a bleeding or oozing site.&nbsp;&nbsp;<\/li><li>&nbsp;When oxidized, cellulose forms a clot quickly as soon as it touches blood.<\/li><li>It is good to use in areas where it is difficult to control bleeding by other methods.&nbsp;<\/li><li>It is not recommended for use on bone as it can inhibit bone <a id=\"post-451-_Int_u0sAqib2\"><\/a>regeneration unless it is removed before closing the wound.&nbsp;<\/li><\/ol>\n\n\n\n<figure class=\"wp-block-embed is-type-video is-provider-youtube wp-block-embed-youtube wp-embed-aspect-16-9 wp-has-aspect-ratio\"><div class=\"wp-block-embed__wrapper\">\n<iframe loading=\"lazy\" title=\"Surgicel Mode of Action\" width=\"580\" height=\"326\" src=\"https:\/\/www.youtube.com\/embed\/nmXFONGfOlM?feature=oembed\" frameborder=\"0\" allow=\"accelerometer; autoplay; clipboard-write; encrypted-media; gyroscope; picture-in-picture\" allowfullscreen><\/iframe>\n<\/div><\/figure>\n\n\n\n<p><strong>Oxytocin&nbsp;<\/strong><\/p>\n\n\n\n<ol><li>&nbsp;Oxytocin may be directly injected into the uterine muscle to cause a contraction during c-section surgery.<\/li><li>It can be injected systemically to help with a hemorrhage from the uterus.&nbsp;&nbsp;<\/li><\/ol>\n\n\n\n<p><strong>Epinephrine&nbsp;<\/strong><\/p>\n\n\n\n<ol><li>Epinephrine is a vasoconstrictor.&nbsp;&nbsp;<\/li><li>It can be used topically within an incision on gelfoam sponges<a id=\"post-451-_Int_uC2aRqrr\"><\/a>. It is commonly used in ear and microsurgical procedures.<\/li><li>Epinephrine can be absorbed systematically and can cause cardiac stimulation. Care must be taken when using epinephrine<a id=\"post-451-_Int_YAwmxtQU\"><\/a>. It is often diluted and only used in <a id=\"post-451-_Int_NZdLtSWs\"><\/a><a id=\"post-451-_Int_sDNqwwD5\"><\/a>lesser amounts.&nbsp;<\/li><\/ol>\n\n\n\n<p><strong>Silver nitrate&nbsp;<\/strong><\/p>\n\n\n\n<ol><li>Silver nitrate comes in the form of applicator sticks. These are applied topically to cause vasoconstriction.&nbsp;&nbsp;<\/li><li>It is commonly used in burns or on nasal and oral surgical sites.&nbsp;<\/li><li>It can help seal areas that cannot be surgically closed and must be left to heal by secondary intention.&nbsp;<\/li><\/ol>\n\n\n\n<p><strong>Thrombin (Tisseel, Floseal)<\/strong><\/p>\n\n\n\n<ol><li>This accelerates the coagulation of the blood and helps with capillary bleeding. It combines with fibrinogen to form a clot.&nbsp;<\/li><li>Thrombin can come as a powder that is sprinkled on a wound.&nbsp;&nbsp;<\/li><li>Hemostatic devices such as Tisseel and Floseal combine Thrombin with other active ingredients to achieve hemostasis.<\/li><li>This accelerates the coagulation of the blood and helps with capillary bleeding. The perioperative team must communicate and work together to recognize when there is substantial blood loss. <a id=\"post-451-_Int_syeeUlAh\"><\/a>The scrub nurse must keep track of the amount of irrigation fluid used during a procedure so that this can be considered when reviewing the amount of blood\/fluid in the suction canisters and saturated sponges. The team estimates the total blood loss and through analysis of blood work, determines if a transfusion of blood product is required.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<div class=\"wp-container-6 wp-block-columns\">\n<div class=\"wp-container-4 wp-block-column\" style=\"flex-basis:33.33%\">\n<h5>When Blood is Needed During Surgery<\/h5>\n\n\n\n<p>Despite the surgical team\u2019s best efforts to obtain hemostasis, there may be a need to transfuse a patient. Depending on the surgical procedure being performed and the risk for blood loss, the patient should be <a id=\"post-451-_Int_nRNkIb34\"><\/a>typed and crossed out before their procedure and a blood transfusion consent should be signed.<\/p>\n<\/div>\n\n\n\n<div class=\"wp-container-5 wp-block-column\" style=\"flex-basis:66.66%\">\n<iframe loading=\"lazy\" src=\"https:\/\/stlawrencecollege-my.sharepoint.com\/personal\/kzekveld_sl_on_ca\/_layouts\/15\/embed.aspx?UniqueId=5bcf9a6f-977c-447a-bb4f-f49ffe20df06\" width=\"640\" height=\"360\" frameborder=\"0\" scrolling=\"no\" allowfullscreen=\"\" title=\"blood consent khsc.pdf\"><\/iframe>\n<\/div>\n<\/div>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h5>Screening before surgery<\/h5>\n\n\n\n<div class=\"wp-container-9 wp-block-columns\">\n<div class=\"wp-container-7 wp-block-column\">\n<figure class=\"wp-block-image\"><img loading=\"lazy\" width=\"2016\" height=\"1512\" src=\"https:\/\/perioperative1001.slc-courses.ca\/wp-content\/uploads\/2022\/12\/word-image-451-23.jpeg\" alt=\"\" class=\"wp-image-472\" srcset=\"https:\/\/perioperative1001.slc-courses.ca\/wp-content\/uploads\/2022\/12\/word-image-451-23.jpeg 2016w, https:\/\/perioperative1001.slc-courses.ca\/wp-content\/uploads\/2022\/12\/word-image-451-23-300x225.jpeg 300w, https:\/\/perioperative1001.slc-courses.ca\/wp-content\/uploads\/2022\/12\/word-image-451-23-1024x768.jpeg 1024w, https:\/\/perioperative1001.slc-courses.ca\/wp-content\/uploads\/2022\/12\/word-image-451-23-768x576.jpeg 768w, https:\/\/perioperative1001.slc-courses.ca\/wp-content\/uploads\/2022\/12\/word-image-451-23-1536x1152.jpeg 1536w, https:\/\/perioperative1001.slc-courses.ca\/wp-content\/uploads\/2022\/12\/word-image-451-23-1200x900.jpeg 1200w, https:\/\/perioperative1001.slc-courses.ca\/wp-content\/uploads\/2022\/12\/word-image-451-23-1980x1485.jpeg 1980w\" sizes=\"(max-width: 2016px) 100vw, 2016px\" \/><\/figure>\n<\/div>\n\n\n\n<div class=\"wp-container-8 wp-block-column\">\n<p><strong>Group and Screen<\/strong><\/p>\n\n\n\n<p>A group and screen <a id=\"post-451-_Int_NctVqGb5\"><\/a>allow the blood bank to:<\/p>\n\n\n\n<ol><li>Determine the patient\u2019s blood group: ABO (0, A, B, AB)<\/li><li>Rh(D) which can be negative or positive<\/li><li>It also allows the blood bank to test for antibodies in the patient\u2019s plasma.<\/li><li>If the patient does have antibodies (positive) they may experience hemolysis if they are transfused with the corresponding antigen positive Red Blood Cell.<\/li><li>In these cases, the blood bank will do further testing to identify the antibodies; for example, anti-D, anti C, anti-K.<\/li><\/ol>\n<\/div>\n<\/div>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<div class=\"wp-container-12 wp-block-columns\">\n<div class=\"wp-container-10 wp-block-column\">\n<p><strong>Crossmatch<\/strong><\/p>\n\n\n\n<ol><li>The results of the group and screen can then be used to cross-match the red blood cells (RBCs) to ensure they are compatible for transfusion.<\/li><li><a id=\"post-451-_Int_1GlLDrUq\"><\/a> The blood sample used to crossmatch a unit of blood can only be used for <a id=\"post-451-_Int_V2TC5RXS\"><\/a>96 hours (about four days) after the first unit of blood is transfused. This is to ensure that no new antibodies have been produced.<\/li><li>For more information, download the following document: Bloody Easy: Blood Administration Version 3.<\/li><\/ol>\n\n\n\n<p>In some cases, the surgical team may not anticipate a need for blood transfusion, or the patient may come into the OR urgently before a proper type and screen or crossmatch can be completed. In these <a id=\"post-451-_Int_S911mZ6M\"><\/a>situations, perioperative nurses must follow the hospital protocol for requesting <a id=\"post-451-_Int_kf4lDu1W\"><\/a>uncross matched blood.<\/p>\n\n\n\n<p>Then the blood bank will <a id=\"post-451-_Int_3nWRCrAO\"><\/a>issue Group O Rh(D) negative <a id=\"post-451-_Int_ERMfRb8M\"><\/a>RBCs for females of childbearing age.
